The AMD Card is a notarized, wallet-sized card that conveys legally-enforceable Health Care Power of Attorney and Advance Medical Directives. The card gives health responders legal permission to contact designated friends and family without violating health privacy laws. Unlike traditional legal documents that are safely stored away, the AMD Card is to be carried at all times, much like one carries his/her credit card, driver’s license, or house keys, ensuring that, in the event of an emergency, the card will be discovered. First responders are trained to look through the personal effects of an unconscious victim. The AMD Card not only identifies the named agent with designated health care power of attorney, the card also reveals any pertinent health history of the patient so that first responders can provide suitable care when the patient is unable to speak for him or herself.

The AMD Card also includes full-length, detailed, electronic copies of the notarized Advance Medical Directive and Healthcare Power of Attorney. These e-documents are sent to the AMD Card member and designated agent so that they can store them on their phones. The e-documents can be shared prospectively with doctors: and put on file at hospitals, colleges, adult living centers, etc. so that in a time of emergency, their existence is known.
